I love templates. Whenever there's a reason (or sometimes when there's not), I use my desktop publishing skills to come up with something I find useful.

This is a basic Publisher file, since most people tend to have Publisher. If so, you can personalize it as you wish. It lists the top allergens as defined by Health Canada with a little space to add more specific information or some of your own. Feel free to amend as you like and if you print it double-sided; you can use the second page which outlines the risk of food allergies. The template is Avery 5392, a 4''x3'' name badge insert, which is readilly available from office supply stores.
